Customized Consultation Forms

Digital disclaimer, waiver, e-sign / Questions for first-time clients / Questions per-service / Pre-treatment advice 'do not eat before' / Allergies, preferences etc.

Allow to customize the client section so I can add all the fields need for a full consultation, therefore all in one place.
i.e. Doctors contact info, In case of emergency contact info, where did you hear about me?, Why have you chosen me?, signature. (maybe photo as well!)
With the ability for marketing questions to be put into a graph i.e. how many have found me via internet or word of mouth.

    A solution most of our users are using currently is to add a Google Forms link and a message to the Confirmation Email template under "Setup">"Client Notifications" > "Confirmations". This way, as soon as your client's book with you, they'll receive an email to fill in your Google Form.

    Customisable consultation forms is a feature that is on our to do list and isn't live yet but here's a link to Google Forms  where you can learn more about how to capture this information digitally.  Hope this helps!

  • I would really like to have this feature however it doesn’t seem to be happening any time soon. Can you provide and updates as to when this is planned? Im thinking if changing to Ovatu as they already have this feature but I love Shedul and really don’t want to move away from you guys

  • I love all things Fresha/Shedul. But I am also thinking of moving to another booking platform that has the customisable consulation forms available, like ovatu or timely. If there is anymore information on when/if fresha are planning on adding this to their service please let us know. I really don't want to have to make the swap.

  • Now with COVID-19, a Liability form that I can create and make mandatory to complete in order to hold an appointment is something I need. I've tried the google form thing, they don't see if, it doesn't send the link to texts only emails. That's not working. Plus I want them to get notified with reminders, if they haven't signed their appointment will be canceled.

  • Could you confirm if this is going to be something provided in the very near future please? I will have to consider moving to another provider if not as there doesn't seem to be anywhere that just provides this as an add on, it seems to be pay for the whole scheduling package to include forms. I don't really want to leave shedul.

  • I've spent hours searching for a form I could link to Fresha but nothing looks straightforward and costs. I may as well pay more for a booking service that includes online consultations if i have to pay for a stand alone form and have the hassle of linking it.
    This is something that is needed desperately . I love Fresha, so easy to use, it would be a shame to have to leave.

  • This really is the perfect time to implement this feature!

    Online consultation and Covid-19 Questionnaire now have to be standard practice so, please can you provide an update?

    Or at least let us know if this is going to be implemented soon / in the next few weeks / months?

    I am sure knowing would help us figure out what to do in the mean time and make a decision while we wait for the additional feature on Fresha.
