Consultation form email to client when patch test booked

When client is bookin patch test email the client consultation form, so they can fill it in and bring with her/him

    Hey Team, 

     

    This is a Great feature and easy to implement. Customer spends 10 minutes till they fill the Consultation form and I guess since time means money we are the ones losing.

    It would be nice to be able to store consultation forms online too (even its for a fee)
